How to get a Home Care Package (Support at Home from 1 November 2025) in Sydney?

  1. To access Support at Home, you first need an assessment by the Aged Care Assessment Team (ACAT). 
  2. This determines your package level and the types of support available. 
  3. To arrange an assessment, contact My Aged Care on 1800 200 422. 
  4. Once allocated, you can use your funding for services such as nursing, respite care,companionship and more, helping your loved one stay safe and independent at home.


Who qualifies for the Support at Home program (formerly known as a Home Care Package)?

Support at Home is available to Australians aged over 65 (or 50 and over for Aboriginal and Torres Strait Islander people) assessed as needing extra care to stay independent at home. Who is eligible for in-home disability supports in Sydney?

To receive NDIS in-home care and community supports in Sydney, you must be an approved participant of the NDIS. 

Eligibility generally requires:

  • Being under 65 years old when applying
  • Living with a permanent and significant disability
  • Being an Australian citizen, permanent resident, or Protected Special Category Visa holder
