Our Helpers provide practical, proactive support that keeps your loved one’s progress steady and predictable to help manage life after surgery or illness:

  • Personal support – We work closely with hospital discharge teams to create a clear, tailored plan for the first days and weeks at home. 
  • Medication assistance – New prescriptions and dosage changes can feel overwhelming. We help families keep everything organised and ensure medications are taken safely and consistently. 
  • Wound and clinical care – Our nurses can manage dressings, monitor healing, and quickly identify concerns, supporting early intervention and preventing complications.
  • Mobility and rehabilitation support – If physiotherapy or gentle exercises are part of the recovery plan, our Helpers can assist with practice, safety, and encouragement so progress feels achievable. 
  • Meal preparation Nutritious meals are essential after a hospital stay. We help with planning, shopping, cooking, and cleanup, so good food is one less thing to worry about. 
  • Light housekeeping – We can support with laundry, bed changes, tidying, and other small tasks that keep the home comfortable and recovery-friendly.

How can I access home help after surgery?

If you’re supporting a family member returning home from the hospital, getting help in place is simple with Five Good Friends.

Here’s how it works: 

  1. Tell us what you need – Complete our short online form to share the situation and the type of support your loved one is likely to need.
  2. Chat with our team – We’ll call within a day, or you can choose a time that suits. Together, we’ll map out the best path to recovering well. 
  3. Find the right Helpers – We’ll recommend skilled Helpers with specific experience, or you can select from our trusted pool of professionals.
  4. Stay connected and in control – All visits can be scheduled and tracked through the Five Good Friends app, giving families visibility and confidence throughout the recovery period.

Technically, there’s no set end date – care can continue for as long as it’s helpful.  

What to expect:

  • Some families only need a few weeks of help for recovery and re-establishing routines.
  • Others continue with longer-term support, adding clinical care, daily assistance, or respite. 
  • As circumstances change, you can adjust your care plan with us so your loved one always has the right level of support.

Transition care is typically designed only to bridge the gap between hospital and home. 

  • The Australian government’s ‘Restorative Care Pathway’ generally lasts up to 16 weeks, depending on the assessment and recovery needs.  
  • Generally, a Restorative Care Pathway requires your provider (like Five Good Friends!) to begin exit planning at the start of a restorative care episode. 

Once this period ends, families can move to Support at Home, NDIS, or private aged care, and we help ensure a smooth, uninterrupted handover. We can also help to prepare a Support Plan Review for the participant to organise this.

Yes, many people can receive government-funded help after leaving the hospital. It ultimately depends on eligibility. 

Support may include:

  • Support at Home’s Resorative Care Pathway for anyone aged 65 and over (or 50 and over for Aboriginal and Torres Strait Islander peoples). 
  • NDIS funding is eligible for those under the age of 65 (or under 50 for Aboriginal and Torres Strait Islander peoples). 
  • Private care options (Full Service or Self-Managed) are available while waiting for approval or to top up services.
