Overview

The End-of-Life Pathway provides up to $25,000 in government funding over 12 weeks to support people who wish to remain at home during their final months. It's part of the Australian Government's Support at Home program and is designed to work alongside state-based palliative care services. 

At Five Good Friends, we've been supporting Members through end-of-life care for years—long before this pathway existed. We understand that this is about more than just services. It's about ensuring your loved one can stay in the place they love, surrounded by familiar faces, with dignity and comfort prioritised at every step.

Our approach is whole-person and coordinated. From the moment you contact us, a dedicated Care Team takes over—mobilising Helpers, nurses, and support services quickly. We work directly with your loved one's GP and palliative care team so you don't have to manage the coordination yourself. Whether it's personal care, companionship, medication support, or respite for family, we bring everything together to meet your loved one's wishes and keep them comfortable at home.

The End-of-Life Pathway provides funding for up to 12 weeks, with flexibility to extend to 16 weeks if needed. Your Care Team will work with you to prioritise what matters most.

Eligibility & referral

Your GP or nurse practitioner confirms eligibility for the End-of-Life Pathway and completes the required form. Once approved, contact us to begin care planning.

Care planning

Our care team contacts you directly to understand your loved one's needs and wishes. We coordinate with their GP and palliative care team to create a personalized care plan.

Support at home

Care begins quickly. Your dedicated care team mobilises Helpers and nurses, manages all coordination, and adjusts support as needs change—so your family can focus on what matters.

Does "Quick" Actually Mean Quick?

Yes. End-of-life situations are our absolute top priority. We usually do the Support at Home assessment on the day you call. Our Best Friend Helpers often start showing up within 24-48 hours. Not weeks. Not "we'll call you back." Days.

You'll need confirmation from a GP or Nurse Practitioner that your loved one has 3 months or less, and they scored 40 or lower on the Australian-modified Karnofsky Performance Status (AKPS). .

But honestly? Just call us. We'll walk you through it on the phone. We'll help with the paperwork. We'll make this as simple as possible while you're dealing with something that's incredibly hard.
