Overview

The Restorative Care Pathway is short-term funding designed to help you regain strength, mobility and ease at home after a setback. It is part of the Australian Government’s Support at Home program and is delivered by approved Restorative Care Pathway providers like Five Good Friends.

This pathway focuses on recovery and prevention — rebuilding function, maintaining independence, and reducing the likelihood of hospital readmission.

Clinical supports, like nursing, physiotherapy, and allied health services are fully funded by the Australian Government for the period of your plan. Participant contributions, as determined by Services Australia, apply for independence and everyday living service categories.

The Restorative Care Pathway offers up to 12 weeks of short-term, goal-based support. It’s designed to help older Australians recover strength, mobility and confidence after illness, injury or hospitalisation. It includes allied health, clinical and personal services, tailored to help you live independently and stay well at home.

You can continue accessing the Short-Term Restorative Care (STRC) program until 1 November 2025. After this date, it will transition to the new Restorative Care Pathway under Support at Home. 

If you’re already in the STRC program, your care will transition automatically. Five Good Friends can answer any questions about what this means for your support.

It’s simple to get started:

  1. Contact My Aged Care to request an assessment. 
  2. Choose a preferred provider for your restorative care pathway (like Five Good Friends!) 
  3. Work with your care partner to create a personalised 12-week plan focused on your recovery goals.
  4. Begin services quickly, often within days of approval.

The Restorative Care Pathway focuses on allied health services such as physiotherapy, occupational therapy and exercise physiology to help you rebuild strength, mobility, and confidence. It may also include clinical and nursing care, as well as personal support while you recover.

Yes. You can access the Restorative Care Pathway twice in a 12-month period if you have been assessed as requiring this support, but not in consecutive quarters.

This ensures you can receive support when you need it — for example, after a hospital stay, surgery or period of reduced mobility — while allowing others to benefit from the program as well.
