Overview

The Restorative Care Pathway is short-term funding designed to help you regain strength, mobility and ease at home after a setback. It is part of the Australian Government’s Support at Home program and is delivered by approved Restorative Care Pathway providers like Five Good Friends.

This pathway focuses on recovery and prevention — rebuilding function, maintaining independence, and reducing the likelihood of hospital readmission.

Clinical supports, like nursing, physiotherapy, and allied health services are fully funded by the Australian Government for the period of your plan. Participant contributions, as determined by Services Australia, apply for independence and everyday living service categories.

What Restorative Care includes

Your plan may include:

Occupational therapy and home safety assessments

Physiotherapy and exercise programs to rebuild strength and balance

Short-term nursing and wound care

Most people receive restorative care for up to 16 weeks, depending on their recovery goals.
