Eligibility for transport under Support at Home depends on your approved funding level, your assessed needs and the purpose of your trips.

For NDIS participants, transport can be included if it helps you achieve your plan goals and is related to your disability. You must be aged under 65 years, or under 50 years for Aboriginal and Torres Strait Islanders, at the time you register for an assessment with NDIA.

Transport can be combined with other Support at Home services to help you remain independent and active in your community. For example:

  • Getting to allied health appointments or therapy sessions
  • Attending social activities or community programs 
  • Supporting everyday errands like shopping or pharmacy trips 

When coordinated with personal care or allied health, transport ensures your care plan is uniquely yours and fits into the life you love.

With Five Good Friends, you can choose between:

  • Full Service Membership – We organise all your transport and care services for you, while keeping you informed. You’ll never be locked in as well, so you can change your transport needs as you require. 
  • Self-Managed Membership – You can use our tools and network to schedule transport and coordinate your support independently, while staying connected with our team for guidance.

As soon as you’ve been assigned a funding level under Support at Home, you can get started with aged care transport services.

Support at Home transport services may subsidise trips for everyday needs, appointments and essential community participation. This does not include general expenses such as:

  • Your driver’s licence
  • Running costs of your own car (fuel, maintenance, insurance)
  • Public transport fares
  • Travel for holidays or non-essential trips
