Support at Home

Introducing our Programs for Living Well

At Five Good Friends, we know that ageing well at home is about more than just care - it’s about living the life you love, for as long as possible. With the right support, you can focus on what matters most while maintaining confidence, strength and independence to keep doing the things you enjoy.

Key takeaways:

  • Our Programs for Living Well focus on preventative health for seniors, helping maintain independence at home.
  • Each program aligns with the Aged Care Standards and delivers measurable outcomes.
  • Five Good Friends Members can access structured, evidence-based care while maximising home care funding.
  • Ongoing support provides greater benefits than one-off services, ensuring long-term wellbeing.

Make the most of home care funding

Our Programs for Living Well are designed to help older Australians and their families maximise independence while staying healthy and connected at home. These evidence-based programs focus on preventative health - building daily habits and routines that support confidence, strength, and wellbeing. They also align with the latest Aged Care Standards, ensuring quality care that meets government guidelines.

Why programs work better than one-off services

While one-off visits to a physio or dietitian can be helpful, real change comes from consistent, ongoing support. That’s the foundation of our Programs for Living Well.

Each 12-week program involves expert clinical guidance tailored to the individual needs of each Member. This can be combined with encouragement and daily support from our trusted Helpers.

Preventative health for seniors: what our programs cover

We’ve started with three key areas where older Australians, and their families, often need support:

Active Life Program: Improve strength, balance, and mobility to reduce falls risk and stay active at home and in your community. Structured support can lower falls risk by up to 30%.

Mealtime and Communication Support Program: Make mealtimes safer and more enjoyable with practical support for posture, pacing, and safe swallowing. Hands-on mealtime support increases engagement and helps maintain meaningful daily routines.

Well Nourished Program: Personalised meal planning and support for better nutrition, energy, and wellbeing. High-quality diets are linked to improved strength, cognition, and a lower risk of dementia.

Independence at home through structured support

Our Programs for Living Well are designed to help you get the maximum benefit from your home care funding. Instead of isolated services, you receive a structured, measurable plan that combines funded supports for real, lasting results. By combining clinical guidance with everyday support, you or your loved one can remain independent at home, with the confidence to keep enjoying daily life.

Why our evidence-based programs work

Quick fixes don’t last. Behaviour change does. Our programs are built around three principles that help both Members and families succeed:

  1. Consistency over intensity – small daily actions create lasting change.
  1. Social support – encouragement from families and Helpers can make routines easier to maintain.
  1. Tracking and feedback – progress reviews ensure each plan is tailored to the Member’s evolving needs and goals.

Live well, live independently

At Five Good Friends, we’re committed to helping our Members live longer, healthier, and more connected lives at home. Our Programs for Living Well are a key part of this mission - combining evidence-based care, compassionate support, and the everyday help you need to thrive. Whether you’re building strength, creating safer mealtime routines, or improving nutrition, you’ll have a dedicated team supporting you every step of the way.

Living well isn't just about adding years to life – it's about adding life to your years, right where you belong: at home, among friends.

1. How do Five Good Friends’ Programs for Living Well make best use of home care funding?

Instead of isolated services, or only responding when challenges arise, these programs put proactive supports in place that deliver long-term benefits in areas such as mobility, nutrition, and safe mealtimes. This structured approach makes best use of the funding you have been allocated based on your care needs, to keep you living in your home for longer.

2. What makes these programs evidence-based?

Each program is developed using the latest research and best practices in ageing well. They align with the Aged Care Standards and are overseen by experienced clinicians to ensure safety, effectiveness, and measurable outcomes.

3. Can these programs really help me stay independent at home?

Yes. Our programs focus on building daily habits, improving strength and nutrition, and reducing risks such as falls. With consistent support and regular progress reviews, Members are better equipped to live independently and confidently at home.
