Self-management is when you manage your Home Care Package funding yourself. It gives you the flexibility and choice to decide what services and supports you need to achieve your goals.

Our self-management plan is ideal for Home Care Package recipients who want to directly manage their own care. You get to decide how you use your funds to best meet your needs and design and coordinate your own care team and schedule.

With a simple tap of the app, let us know exactly what support you need and we’ll get back to you within 24 hours with Helpers who closely match your criteria. You can then compare their profiles, send them a request, and chat with them directly to organise and book-in a visit.

When you look for a Helper remember to think about what matters to you. You can compare Helper profiles which tell you all you need to know, including their background, skills and interests. If you’re interested in any of them, you can send them your request and if they accept your invitation you’ll be able to chat with them directly and organise to book-in a visit.

It runs quietly in the background analysing check-out notes left by your Helpers and your approved care network to identify any risks or concerns. These observations raised by the software notify your care team so they can advise whether you need additional or proactive supports.
