Wellbeing check-ins.
A wellbeing check-in is a short phone call that provides reassurance and remote supervision.
When we onboard a new At Hand customer, we'll conduct a short interview to assess their key interests, activities and health concerns. This information will be stored in that customers profile and will be used to provide context and personalisation to each and every call.
Where possible, we will aim to match a customer with same individual conducting the wellbeing calls to ensure they can develop a relationship and rapport over the phone throughout multiple calls. Five Good Friends will share key updates and notes with these helpers via their profile.
Each check-in will also include a few key health indicator questions. The questions we ask have been designed by our Care Management team to help keep track of the overall wellbeing of our customer.

I'm already a Five Good Friends Private Member and am interested in this pay as you go plan instead. Can I transfer to At Hand?
Private Members with Five Good Friends can transition between their existing membership level and At Hand - however they must be aware while they are receiving our At Hand, we can no longer guarantee they will remained matched to their existing Helpers. For the duration you're on an At Hand you will no longer be eligible to receive the following services:
- Care Management
- Clinical Services
- Helper Matching
- Help Plan & Reviews
- Package Management (if you're a Home Care Package recipient)
- Allied Health Management
While they are on At Hand.
Home Care Package or NDIS Supported Care?
At Hand is designed to be a low cost, entry level option for private paying customers looking to add a small amount of supervision and services. It is not an appropriate level of care for those receiving funding under the HCP or NDIS systems. We have other types of membership for those cases, that include:
