Five Good Friends - At Hand Membership

At Hand when you need us most.

Wellbeing check-ins to provide comfort and combat isolation.

Wellbeing check-ins.

A wellbeing check-in is a short phone call that provides reassurance and remote supervision.

When we onboard a new At Hand customer, we'll conduct a short interview to assess their key interests, activities and health concerns. This information will be stored in that customers profile and will be used to provide context and personalisation to each and every call.

Where possible, we will aim to match a customer with same individual conducting the wellbeing calls to ensure they can develop a relationship and rapport over the phone throughout multiple calls. Five Good Friends will share key updates and notes with these helpers via their profile.

Each check-in will also include a few key health indicator questions. The questions we ask have been designed by our Care Management team to help keep track of the overall wellbeing of our customer.

Monitoring by intelligent technology

We integrate our technology, Lookout™, into all aspects of the way we do care. Lookout™ analyses our wellbeing check-ins and survey, alerting us to risks or changes in health and wellbeing. That way, we can identify health changes and act upon signs of concern. This means you get a smarter, coordinated and more proactive service to give you that extra peace of mind.

How does Lookout™ keep me and my family safe?

Five Good Friends goes beyond a traditional home care provider. Sure, we can help with things like meal prep, medications or grocery shopping. But we also go a level deeper by assessing your health risk factors, using technology to help manage your health, well-being and daily routines, and creating a plan to ensure you remain living in the home and community you love.

The care, time and patience that everyone puts in with my mother is unbelievable. Their responsiveness and being able to contact about any issues is excellent. Highly recommended. Great team work.

I find it so reassuring to know that support is just a phone call away. Last week I needed transport to an eye specialist in the city. My son was unable to help on that day. At short notice I turned to Five Good Friends. Charming Elise drove me there and back. I did not need to be exposed to anyone else and felt very safe.

Mum and Dad are more relaxed and happy already. The helpers are so kind, gentle and knowledgeable. Everyone has been easy to get ahold of, transparent and good at answering the questions we have up front. Couldn't be happier!

My father's Helper is like a friend to my Dad, assisting him to do the things he loves to do and some new ones too. She works with me to help improve my father's mental and physical health, whilst communicating with me always. I really appreciate the caring help and assistance from Five Good Friends.

I have become very fond of my helper. She saves me a lot of work, and in particular she takes me shopping - something that I was having a lot of trouble with before, but no longer. Thank you.
