What is a care-managed plan?
Care-managed plans provide our member's with security and oversight of their care delivery through connection to a care team.
Care-managed plans best fit member's who are looking for assistance from professionals in the management and oversight of their home care services. This may be because the member and their family is unfamiliar with the many systems involved in coordinating home care or because their care involves complex health planning that requires professional assistance to manage.
Member's on care-managed plans are supported by a care team comprising of:
- Care Managers - are here to ensure the care and help we provide is aligned to your goals and changing needs.
- Community nursing consultants - who are here to provide nursing help and support.
- Helper coordinators - who will ensure you have the best Helpers matched to you providing the same consistent help and care day in day out.
- Helpers - who provide the day to day support and help in your home.
Care-managed member's enjoy the benefits of our remote care monitoring with observations continually monitored by the care manager in their team. The care manager will respond to alerts with responses and recommendations suitable for that member's needs and the urgency of the situation.
Five Good Friends workforce and helper coordination team manage the matching, rostering and adjustment of visits. Our workforce team will work tirelessly to find you the perfect match from our existing set of verified Helpers or recruit a new Helper in your area if no-one suitable can be found initially.
Our clinical nursing and care management team will work with care-managed member's to ensure they are set up with a help plan and budget at sign-up that is regularly reviewed to ensure quality and affordable delivery of services.
Suitable for members who have been approved for a commonwealth government funded Level 1-4 home care package that are looking to work with a full care team to plan, managed and oversee their care.
Suitable for member's who are able to privately fund their home care that are looking to work with a full care team to plan, managed and oversee their care.
What is a self-managed plan?
Self-managed plans are all about providing member's with the tools and opportunity to manage key aspects of their own care. Self-managed members or their authorised representative are capable of safely taking control of the day-to-day management of their home care.
Member's on self-managed plans will be free to manage tasks such as:
- Co-ordinating the roster and visit schedule directly with matched Helpers.
- Selecting available Helpers via the Five Good Friends app.
- Managing the finances on their account utilising our statements feature.
Self-managed member's with Five Good Friends still enjoy the benefits of our remote care monitoring with observations continually monitored by our team. We will ensure that alerts raised by these tools are shared with member's and their authorised representatives alongside recommendations for how to best respond.

What is an NDIS plan?
NDIS plans are all about choice and control for our participants. We believe in capacity building. We want you to spend more time (and more of your budget) focused on achieving your goals. We help you do this by providing a supportive team and fantastic software to make administration and coordination simple, straightforward and understandable for everyone.

Can I have both?
Yes - the services do not impact each other however you will deal with a seperate care manager and support coordinator for each service.
What is an At Hand plan?
At Hand plans provide simple ways to check-in and monitor the wellbeing of member's that provides reassurance and remote supervision.
When we onboard a new At Hand member, we'll conduct a short interview to assess their key interests, activities and health concerns. This information will be stored in that member's profile and will be used to provide context and personalisation to each and every call.
Where possible, we will aim to match a member with same wellbeing caller throughout to ensure they can develop a relationship and rapport over the phone throughout multiple calls. Five Good Friends will share key updates and notes with these helpers via their profile.
Each check-in will also include a few key health indicator questions. The questions we ask have been designed by our Care Management team to help keep track of the overall wellbeing of our members.
