The Help Plan – a living document that enables fantastic outcomes.

At Five Good Friends, we strive to help people live engaged and successful lives in the homes and communities they love. We put our Members and their families at the centre of everything we do.

Our Members talk and we listen.

That’s why they only need to tell us their story once. A Members’ story, goals, and expectations are captured in a living document known as the Help Plan. Learn about how to access your Member's Help Plan here.

For Helpers, the Help Plan is critical.

The Help Plan outlines what’s expected of us, what we should be doing with and for Members, as well as anything we need to be aware of to deliver great outcomes for Members and loved ones. It also provides professional boundaries which ensures you know what is expected as part of your role as a Helper. Everyone is on the same page.

When someone becomes a Five Good Friends member, we encourage them to take the time to sit with family or loved ones to discuss their expectations and support needs. This planning will ensure that they tell us everything we need to know about how they want to receive care and support.

Every time a new Helper comes on board to care, one of the most important part of your preparation is to look at the Help Plan.

For a Five Good Friends Member, spending time on their help plan and what they want is important.

It tells Helpers a little bit about who they are, likes and dislikes and religion, culture and identity. The plan also provides practical information about living arrangements, how we can access the home to ensure a safe experience for Helpers and Members.

The plan gives Helpers a great place to also understand what is important.

My Member wants to change their help plan?

Help plans are important and we know your circumstances and needs change over time. It is important that Members are part of writing the help plan - the best help plans are the ones constructed in collaboration with family and loved ones.

If your Member wants to make minor changes to the help plan, this can be made by speaking with Member Services team. Circumstances change, we understand that.

Once a Members signs up they then have an annual review scheduled. If however, they need to update it sooner remind them to please contact our Member Services team on 1300 787 581.
